The Patient Information Sheet tells you about why we are doing the study and what we are planning to do. At the end of this document is the Consent form, which shows the questions we will ask when we seek to receive your consent to participate in the study. This will be taken in person, so you have a chance to ask any questions or learn more about what we are doing.
